Many accidents are preventable if anticipatory precautions are put in place.  The recent Hampi Express tragedy in which 25 passengers, many of whom women and children, yet another example to sins of omission by railways.  Since there is no fail-safe mechanism 'a couldn't care less' attitude of the relevant authorities on safety aspects is a regrettable failure on their part.  The probing question that they need to receive their attention is what was the driver's state of mind at the time of accident if it's proved to be a human error.  Sad to say, safety precautions in every sector are zero where human lives are concerned.
